Stolen Devices Contained Data, 2,700 Senior Health Partners Members Notified

New York-based Senior Health Partners (SHP) is notifying roughly 2,700 members that a laptop and smartphone containing their personal information was stolen from the apartment of an assessment nurse employed by Premier Home Health, a business associate.

How many victims?
Roughly 2,700.

What type of personal information?
Names, addresses, Social Security numbers, Medicaid ID numbers, dates of birth, phone numbers, medical services rendered, diagnoses, and health insurance claim numbers. Medicaid plan, eligibility, and program information, as well as third-party administrator information, was accessible for one individual.

What happened?
A laptop and smartphone containing the personal information of SHP members was stolen from the apartment of an assessment nurse employed by Premier Home Health.

What was the response?
SHP is reviewing and updating its policies and procedures, and those of its business associates. All impacted individuals – as well as family members and caregivers of the affected members, to the extent possible – are being notified, and offered a free year of identity and credit monitoring and restoration services.

On Nov. 26, 2014, a laptop bag containing a laptop and smartphone was stolen from the apartment of an assessment nurse who worked for Premier Home Health. Premier Home Health notified SHP of the theft on Dec. 10, 2014. The laptop was password protected and encrypted, but the encryption key was stolen with the laptop bag. The smartphone was not password protected and not encrypted.

“SHP and Premier conducted investigations and found no indication that anyone accessed or misused any personal information or protected health information,” according to a release.
